In my classes I like to focus on the necessities of each student, that is why I always create a personalized content plan. Some people may want to improve their writing skills, so I would highlight grammatical and ortographical issues. Whereas others prefer to have lessons in order to improve their conversational skills, then I would dedicate more time to speaking activities.
Regarding the length of a lesson, I consider 1h class is the mininum to cover a specific topic. But of course you will need to invest more time to learn the language. Personally, I recommend at least 2h a week taking classes and then 2 more hours at home (it can be watching a movie in the language, listening to songs or just reading).
Finally, as I mention in my profile, I have teaching experience since I was 16 (almost 10 years ago) and I had students from a huge variety of ages and levels. Here in Spain I am an English teacher but I also have previous exposure to teaching Spanish as a second language. In fact it is easier for me as it is my native language.
