I usually begin sessions by having students tell me in their own words about the topic we are working on. This way, I can gauge their understanding and see what information we can examine in greater detail and work on to improve.
High school students
Hire a teacher you are comfortable with and who you can communicate freely with! The hardest part of learning is admitting when you need help, so make sure your teacher is someone you trust will understand your needs.
Not only should good teachers want their students to be open and honest about what they do and do not understand, but they should also make sure that their students know this as well.

A few things worth thinking about before reaching out
When it comes to identifying what topics students need help with, I think they should rank these topics according to how well they currently understand them and if they think that they may be able to understand it on their own with some practice. All topics and the ranking should be mentioned to their teacher, regardless.

Please note that this rate may change depending on how intense and thorough the lessons will be (Is the student in need of practice and review of topics? Or do they need an entire lesson plan?).
